Our team works collaboratively with your organization to use leading assessment tools for understanding the strengths and opportunities for improvement in your medication system. An action plan can be developed to help prioritize and start key improvements.
Proactive Risk Assessment using Failure Mode and Effects Analysis (FMEA)
Our team works collaboratively with your organization to complete a structured proactive assessment of a specific process related to medications. The goal is to map the process, identify potential failures, and make changes using human factors principles to reduce the risk of the failures and prevent harm to patients and residents. A summary report is provided to support your organization in taking action.
Community Pharmacy Medication Safety Consultation
Through a series of virtual meetings, our team of medication safety experts will review dispensing and related patient care processes to identify possible system-based risks. Our team works collaboratively with a pharmacy manager, pharmacist, or pharmacy technician supervisor, as well as indirectly with their community pharmacy team to complete this process and provide a summary report. We will help with understanding and applying the principles supporting safe medication practices, including basic human factors, systems approach and just culture.
Provincial pharmacy regulatory authorities periodically use this consulting service to support a remedial approach as part of their investigation and/or discipline processes.
